These are very comfortable ball gags. If you've ever used a ball gag with a leather strap, that doesn't have rounded edges it does dig into the corners of your mouth, and can be quite painful. The silicone straps on either side of the ball do allow you to "cinch" the ball in tighter with more comfort.
It feels like a ball, like maybe the size of of ping pong ball up to maybe a golf ball size object, in one's mouth. Sometimes they're shapes besides round, like little hearts or whatever. They can be slightly soft, if made of silicon, some are just hard plastic. It makes it hard to speak articulately, though you can still can make sounds and maybe kinda talk depending on exact fit.
They do feel uncomfortable in general, because your jaw is being stretched into an unnatural position. IMO that's a feature, not a bug. ;) Smaller and squishier balls are less uncomfortable than big and hard balls, and be careful not to get those with sharp straps that cut into the sides of your mouth. That's just the wrong kind of pain, in my humble opinion.
Not so sure how I feel about a metal ball...bite down hard enough and your teeth are all chipped Then again I have never had one in so...
Conversely I do not believe there is a ball gag out there you can't push out of your mouth to some degree. Your jaw is extremely mobile. Push you jaw forward, now push your jaw backwards. There's a lot of room. That room still exists with a ball gag, and no matter how tight you pull a strap, you can still push it around to an extent.
