The appeal of tunnel plugs for me is the fantasy of being able to see inside and being able to insert things through the tunnel. In all honesty, it's better in the fantasy than reality since the diameter of the tunnels usually isn't very big, so you have to be at the right angle (and have a light) to be able to actually see inside.
Generally they're designed to angle towards your front, the idea is that they press against your prostate to give some extra stimulation. But, if you find it more comfortable the other way theres no harm.
